Consumer Health Data Privacy Policy

Consumer health data we collect and why

• Information you share when you contact us, by contact form, email, phone or a free connection call, such as symptoms, injuries, pregnancy or postpartum status, pelvic health concerns and goals. We use it to answer your questions, decide whether our services fit your needs and schedule a visit.
• Booking and intake information you enter in Jane App, such as health history, current symptoms, conditions, medications, pregnancy and postpartum information, and the services you book. We use it to prepare for and provide your care.
• Clinical information created during your care, such as evaluation findings, treatment notes, progress measures and home programs. We use it to provide, document and coordinate your care.
• Payment and billing information related to the health services you receive. We use it to process payments and prepare superbills you can submit to your insurer.
• Website activity that may suggest an interest in health services, such as visits to our pelvic floor or pregnancy pages. We use it only in aggregate to run and improve our website, not for advertising.

Sources of consumer health data

• Directly from you
• From our service providers when you use them, such as Jane App and Squarespace
• From other health care providers, when you authorize them to send us records
• Automatically from your device when you visit our website

Consumer health data we share

We may share any of the categories above, only as needed to provide the services you request or with your consent.

Third parties we share with

• Service providers (processors) that act on our behalf: our scheduling, clinical records and payment system (Jane App, and its payment processor), our website host (Squarespace), and our email and phone providers
• Other health care providers, when you ask us to share information with them
• Government authorities or others, when required by law

We have no affiliates that receive consumer health data. We do not sell consumer health data.

Your rights

If you are a Washington consumer, you have the right to:

• Confirm whether we collect, share or sell your consumer health data, and access that data, including a list of the third parties we have shared it with and their contact information
• Withdraw your consent to our collection and sharing of your consumer health data
• Ask us to delete your consumer health data

Some information, such as clinical records we must keep as your physical therapist, may be governed by other laws that require us to keep it, including chapter 70.02 RCW. If that applies to your request, we will tell you.

How to make a request

Email lindsay@alpenglowptseattle.com with the subject “Health data request,” or call 425-780-7533. We may need to verify your identity before acting on your request. You do not need to create an account.

We will respond within 45 days. If we need more time, we may extend this once by up to 45 more days and will tell you why within the first 45 days. Requests are free up to twice in any 12-month period.

How to appeal

If we decline your request, you may appeal by replying to our decision or emailing lindsay@alpenglowptseattle.com with the subject “Health data appeal.” We will respond in writing within 45 days and explain what we did or did not do and why. If we deny your appeal, you may file a complaint with the Washington State Attorney General.

Changes to this policy

If we plan to collect, use or share consumer health data in a new way, we will update this policy and ask for your consent first where required.
